Please use this form to order our new embroidered Community Church apparel items.  When completed, place the order form along with a check, payable to Community Church with “church apparel” in the memo section.  You may also pay cash; but please use an envelope and mark it “church apparel.”   Return your order form along with your payment to the church or place in the offering basket.

After we receive a minimum of 12 or more items (from all order forms), we will send them in for processing.  They will usually be available in 10 working days and brought to the church for delivery.
